General Statement Of Responsibilities

The Indian Child Social Services Family Support Partner provides prevention case management and supportive services assisting with strength-based wrap around services to families in need. The Family Support Partner will coordinate with Southern Indian Health Council Departments, San Diego County Health and Human Services Agency (HHSA), and Consortium tribes to provide information, resources, and referrals to prevent the breakup of Indian Families.

Specific Duties And Responsibilities

  • Provides in-home parenting education utilizing evidence-based curricula, case management, and support to families with children at risk of abuse or neglect.
  • Conducts on-going assessments regarding child abuse and neglect.
  • Coordinates closely with Child Welfare Services and other services providers.
  • Works closely with Indian Child Social Services Coordinator and HHSA to prevent child abuse through culturally based education, referrals, and assessments in the tribal community.
  • Works with tribal communities, tribal schools, and community school districts to address community referrals by providing services, educations, assessments, referrals to crisis counseling and developmental screenings.
  • Reduce isolation of family and youth in the program through cultural and community activities, services, and support.
  • Conducts community presentations on child abuse awareness and prevention, program activities, and services available.
  • Support foster families, relative caretakers, and assist with recruiting foster homes, which includes navigating the Resource Family Approval (RFA) application and Tribally Approved Home (TAH) process, provides transportation and delivery of program supplies to ICSS families.
  • Responsible for identifying family’s strengths and needs, provides assistance with any immediate needs in order to develop a service plan.
  • Able to assist case manager with referrals for ICSS clients to necessary internal and external agency services.
  • Maintains timely documentation and reports and is responsible for documenting all client interactions in a professional manner.
  • Collaborates with all other SIHC Departments to reduce isolation and enhance resiliency, keeps current with community needs for services and prevention health education to recognize the mental health conditions and break down barriers to access services, works closely with Kumeyaay Family Services including therapists, prevention counselors, domestic violence advocates, and substance use counselors to share resources and work in a Multi-Disciplinary Team (MDT).
  • Attends and contributes to Case Consultations, Child Protection Team (CPT) meetings to support client needs, address community prevention referrals and promote resources.
  • Database entry into case management software system, documenting all client interactions in a professional and timely manner and completing all the necessary paperwork to promote internal communication with all ICSS staff, maintaining accurate information.
  • Works closely with the ICSS Program Coordinator and Case Manager to provide child abuse prevention through education, referrals, and assessments and in the tribal community.
  • Working knowledge of agency policies and procedures, AAAHC, HIPPA, confidentiality, Health and Human Services, and other county, state, Tribal, and federal regulations.
  • Ability to work a flexible schedule may be required based on need.
  • Other duties as assigned fit the needs of the Grant, Department, and SIHC.

Qualifications

Education/Experience: B.S/ B.A degree in Social Work or related field and/ or two years of related work experience in case management and direct client services. Working knowledge of the Indian Child Welfare Act including requirements and guidelines is preferred. Must have good verbal and written communication skills, as well as good stress management and organizational skills. Must have knowledge of responsibilities involved in doing work that is highly confidential including but not limited to the purpose of conducting home visits. Ability to establish and maintain effective working relationships with clients, staff, and the community. Experience, conducting effective community presentations and facilitating meetings will complement your application.

Possess abilities to work flexible hours as required to meet department needs; facilitate and negotiate and resolve issues professionally; work collaboratively in a team approach; effectively communicate with confidence and knowledge to staff and clients.

Applicant must be able to adapt to changing situations and circumstances and handle pressure with dignity, in all circumstances maintaining confidentiality. Possess an ability to always act professionally in the workplace and the community.

Licenses/Certifications: A valid California driver’s license required with application submission and must be maintained throughout employment. Current certifications and/or licenses appropriate to the position’s required education and profession.

Applicant should be sensitive to client’s needs and knowledgeable about the local Indian community.

Other: Applicant must successfully pass a pre-employment tuberculin skin test or x-ray, and blood/urine drug screening test. Health must be adequate to perform all duties of the position. Applicants must complete an SIHC application questionnaire and submit it to Human Resources prior to the close dates indicated. The position is primarily responsible to the Alpine clinic although local travel may be required.

Physical and Personal Requirements: Normal clinic/office environment. Sit or stand for long periods of time. Reach, bend, climb, stoop, lift up to 25 lbs. repetitive hand movement; use and view PC. Reliable transportation and car insurance as required by the state. Travel as needed.

Compliance:Each employee plays a key role in creating a compliance culture at SIHC. Employees are expected to learn and comply with all SIHC policies and procedures. One must have the ability to understand the implications and complexities of all compliance policies. Participation in all compliance training is mandatory, and generally have a heightened awareness of policies and regulations that are specific to one’s department and position. All SIHC employees are expected to report violations or suspected violations of compliance policies or regulations.

Indian Preference

INDIAN PREFERENCE: In the filling of any SIHC job vacancy, preference may be given to qualified Native American Indians, pursuant to the Indian Preference Hiring Act, 25 USC 472, unless other laws require the filling of a vacancy without regard to Indian preference (e.g. Public Law (P.L.) 94-437, “Indian Health Care Improvement Act (IHCIA). To receive Indian preference for any SIHC position, the applicant must be enrolled (or be eligible for enrollment) as an American Indian with their Tribe, or must be certified as an American Indian from their designated Tribal Representative. Applicants claiming Indian preference must attach verification of their claim to the SIHC application, including Certification of form BIA-4432, which is available from the SIHC Human Resources Department. If verification is not or cannot be verified, the applicant will not receive Indian preference for purposes of the interview.
