Southwestern Behavioral Healthcare is currently seeking a full-time Health Services Responder to add to our team of professionals. This position will provide services to individuals in crisis as part of the integrated Crisis Receiving and Stabilization Team. Additionally, the Health Services Responder will provide screening, assistance, and guidance for minor medical issues to individuals making contact with Southwestern’s Crisis Services program.

WHAT IS THIS POSITION RESPONSIBLE FOR?

  • Monitoring on-site visitors including providing ongoing room checks and risk assessment, engagement and motivational interviewing.
  • Providing health screening and triage for individuals in crisis, including vital signs.
  • Responding promptly when individuals experience a crisis that puts them at great risk for hospitalization or other higher, or less desirable, out of home levels of care.
  • Responding to community crisis on site in the community (shelters, medical facilities, businesses, public places or client homes), if safe, with possibility of co-response with law enforcement or other first responders; crisis response also could occur via phone or Zoom.
  • Assessing crisis through risk assessment, management, crisis prevention, intervention and engaging as appropriate to help de-escalate crises and promote stability and recovery.
  • Collaborating with psychiatrist, nursing team, and other medical providers, for provision of medical care during crisis recovery.
  • Providing emergency interventions within scope of practice, independently and as directed.
  • Maintaining an orderly stock of medical supplies as needed; correctly documents accounting for medications temporarily stored.
  • Engaging and collaborate with community partners, to include emergency responders and community social service agencies to coordinate transitions of care for individuals in crisis.
  • Providing quality clinical documentation and being present for all services provided.
  • Advocating on behalf of clients and families served.
  • Collecting and entering data for ongoing program evaluation and improvement.
  • Monitoring and documenting any medication brought by visitors to the Crisis Stabilization Unit.
  • Providing guidance and direction on the treatment of minor illnesses and injuries.
  • Interacting with individuals initiating contact with Crisis Services through the chat (TAWK) feature.
  • Contributing to a welcoming and supportive care environment through good customer service, good communication skills, and professionalism in team interactions.

  • Demonstrating flexibility and adaptability in all aspects of the position.
  • Maintaining positive relationships with community partners.
  • Utilizing electronic medical record for clinical documentation.

WORKING ENVIRONMENT:

This role routinely uses standard office equipment such as computers and phones. Working conditions may include exposure to adverse weather conditions and unfavorable health and safety conditions such as pets, rodents, insects, unclean environments, foul smells, animal waste, drug paraphernalia, and hostile and/or threatening individuals.


MINIMUM QUALIFICATIONS:

  • Current Medical Assistant Certification through American Association of Medical Assistants (AAMA) required, or Licensed Practical Nurse (LPN). Associate degree or completion of 2 year certification program required.
  • Must possess interest and ability to work with community members who are in crisis, and have severe and persistent mental illness
  • Experience with working with community partners and the homeless population preferred
  • Obtain and maintain Non-Abusive Psychological Intervention (Welle) certification.
  • Must be at least 21 years of age and maintain a valid driver’s license.
