Job Description

General Purpose

Responsible for oversight of day-to-day operations of Sparrow Home Care. Provides leadership to management team and all Caregivers to assure operations and the care delivered is consistent with the mission and vision of Sparrow Health System. The Director is responsible for achieving agency goals, maintaining a safe care environment for all patients, achieving fiscal targets, and ensuring compliance with regulatory guidelines and standards.

Essential Duties

  • Manages, standardizes, and aligns operations to industry best practices to achieve strategic and financial goals.
  • Maximizes operating and financial performance through effective expense management, staffing management, enhanced productivity, and increased market share.
  • Ensures compliance with State, Federal (CMS Conditions of Participation) and Joint Commission guidelines and standards.
  • Identifies key measures for quality and safety, maintains and tracks these indicators, updates areas of focus based on results, develops action plans when indicated, and communicates to key stakeholders.
  • Develops and maintains policies, procedures, and standing orders in accordance with health system and industry best practice to ensure operational standards and regulatory compliance.
  • Plans, organizes, and allocates clinical staff to carry out the activities necessary to accomplish agency objectives.
  • Establishes the agency's strategic plan to best meet the goals of Sparrow Health System.
  • Acts as the agency representative to community programs, functions, and professional organizations.
  • Establishes relationships across the care continuum to ensure coordinated, high quality, patient care.
  • Keeps Sparrow Community Care Board of Directors, Sparrow Health System Quality Committee, Professional Advisory Committee, and other defined committees apprised of agency standing.
  • Establishes clear, concise, and mutually agreed upon expectations for Home Care Caregivers and provides feedback regarding individual performance.
  • Creates an atmosphere that supports and encourages high performance, teamwork, and continuous quality outcomes.
  • Participates in MNA labor meetings, negotiations, and necessary activities.

Job Requirements

General Requirements

Valid Drivers License


Work Experience

Minimum of 3 years Home Health Care experience and 5 years healthcare experience. Minimum 3 years management experience. Previous experience in home health leadership/administration. Extensive computer skills with exposure to multiple computer programs. Director-level or above experience preferred


Education

Bachelor’s degree in Nursing. Master's degree in Public Health, Business, Nursing or Health Care Administration preferred


Specialized Knowledge and Skills

Demonstrated skills in preparing and implementing effective strategies for operational performance Demonstrated success in preparing annual financial plans Leadership/teambuilding/coaching/mentoring Policy/procedure/contract administration Clear presentation skills which influence others Problem identification and resolution
