Special Tree is located in Romulus, Michigan. Our Neuro Care Center sits on our 23-acre campus which includes a 39-bed sub-acute unit, full-service therapy clinic, indoor pool, state-of-the-art greenhouse, and much more.
Special Tree is Seeking a Social Worker for its Long Term Care Facility
In the spirit of Fabulous Everyday Care, Every Day, the primary purpose of the Social Worker role is to address the psychosocial needs of clients and their families, assist clients and families in adjusting to the rehabilitation process and extended care living, assist clients in maintaining their rights, and work in conjunction with the care team, family, and community in providing opportunities for restorative or adaptive rehabilitation.
The Position
- Identifies medically-related social and emotional needs of clients and families, and develops and implements programs/ services to meet those needs.
- Assists with admission and discharge procedures. Documents Social Work activity to meet the needs of the interdisciplinary team, the client, and the family. The Social Worker also maintains confidential records for sensitive material not appropriate for the client’s chart.
- Manages crisis intervention with clients and families as appropriate. Provides information to clients and families regarding Advance Directives, Guidelines for Life Sustaining measures and Client Rights.
- Provides client, family, and staff education and training as needed and documents appropriately.
- Participates in the MDS, RAI, RAP process.
- Participates in Care Conferences and Team Meetings and communicates with members of the team concerning client treatment as necessary.
Qualifications
- We're looking for full-time Social Worker in the State of Michigan to coordinate the care to our clients with Spinal Cord and Traumatic Brain Injuries.
- Bachelor's Degree in Social Work from an accredited college or university
- Professional license as required by the State of Michigan
- Experience and/or educational background in long term and in subacute care.
- Experience with Medicare
- Must possess the ability to make independent decisions and the ability to deal tactfully with coworkers, residents, family members, visitors, government agencies, and the general public
- Academy of Certified Social Workers credential preferred
