Hospice Case Manager RN
Job Posting for Hospice Case Manager RN at Spero Hospice
The Registered Nurse (RN) Case Manager based in Essex County, New Jersey is responsible for delivering high-quality and compassionate care to patients and their families.
Responsibilities:
- Assess patient condition and monitor progress
- Develop and implement a plan of care
- Collaborate with the Hospice Team in an interdisciplinary setting
- Educate and support patients and their families
- Communicate with other medical professionals
- Understand and adhere to regulatory and clinical documentation guidelines
- Ability to work independently and as part of a team
Qualifications:
- Possession of a valid Registered Nurse (RN) license in New Jersey
- Bachelor's degree in Nursing (BSN)
- At least one year of experience in a Hospice setting
- Previous experience in hospice care is an advantage
- Medical and surgical experience of at least one year is a must
- Outstanding communication and interpersonal skills
Job Type: Full-time
Pay: $43.00 - $45.00 per hour
