- Develops goals consistent with the team treatment plan, further operationalizing a plan for improvement and a strategy for present and future changes or progress.
- Meets with patients (in-person, virtually or telephonically) to discuss goals, progress, and challenges. Implements goals using motivational interviewing techniques.
- Helps patients understand their "why" for change.
- Empowers patients to see themselves as the experts on their own health and circumstances.
- Uses a broad knowledge of health and wellness to help patients navigate health concerns, with a specific focus on coaching in lifestyle strategies for chronic disease prevention and/or slowing of disease progression. Understands commonly used biometric measures.
- Recognizes potential imminent danger and medical red flags, knowing when and how to refer to another health care professional. Remains alert to the need for a behavioral health or care management referral.
- Identifies social, emotional, or environmental barriers to change.
- Is a subject matter expert in self-management of chronic disease.
- Provides support and accountability for goals.
- Has a basic understanding of common chronic diseases and conditions including hypertension, diabetes, obesity, cardiovascular disease, high cholesterol, and metabolic syndrome
- Light local travel may be required.
Minimum Qualifications
- Nationally Board-Certified as a Health and Wellness Coach (NBC-HWC) within 18 months of employment.
- Bachelor's Degree in a health-related field (examples such as Health Education, Exercise Physiology, Psychology or Dietetics).
- One to three years of prior experience working as a health coach or related experience in health education or social sciences/social services; or any combination of education and experience, which would provide an equivalent background.
- Proficient in motivational interviewing techniques.
- Proficient in Microsoft software (Word, Excel, Outlook, etc.)
Preferred Qualifications
- Master's Degree in a health-related field.
- Understanding of disease management principles preferred.
- Familiarity with electronic medical record charting.
