Position Title: Case Manager - Champaign & Bloomington Salvation Army Grant Per Diem (GPD) Program
Report To: GPD Bridge Program Manager
Qualifications:
Education: Bachelor’s degree in social work, public health and/or human services and/or previous experience working with the homeless population and/or working with Veterans and in-depth knowledge of VA programs and guidelines.
Experience: Outgoing personality with good communication skills and the ability to work with diverse cultures is necessary. Experience in working with the Veteran population and have experience in area of substance abuse, Veteran assistance and PTSD is beneficial. Strong leadership style and good organizational skills are required. The individual must obtain CPR/Blood Borne Pathogen training within 90 days of employment. Having knowledge of The Salvation Army, Federal and State Government, and community resources is beneficial. Must have valid driver’s license and have the ability to obtain clearance through the Army’s insurance carrier; must undergo a criminal background check and attend The Salvation Army’s “Safe from Harm” training. Individual must be willing to become familiar with and support the mission of The Salvation Army.

Summary:
Provide supportive services at the Champaign and Bloomington GPD sites using the rapid re-housing strength-based case management approach to Veterans and their families with a desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives through sustainable permanent housing interventions.
Essential Functions:
Provide supportive services using strength-based case management, Housing First and Rapid Rehousing Models. Provide assessment, critical time intervention, linkage and referral to community and veterans services to Veterans and their families who are homeless and have accepted a permanent housing intervention. Conduct outreach in the community in order to identify and screen eligible participants. Establish case plans based on primary goals of housing stability, increased income/financial resources and skills, and self-determination.
- Conduct outreach and engagement activities with GPD staff and other Veteran service providers in order to identify and support participants for GPD services. Engage and rapport with the target population and meet the desired outcomes of eligibility requirements.
- Participate in Stand Down, in annual CHALENG and other VA/community outreach activities.
- Collaborate with local leadership and the VA GPD liaison to best support clients and their self-determined goals.
- Conduct screening interviews with potential applicants in accordance with GPD eligibility definitions, including Veteran discharge status.
- Provide appropriate referrals for veterans/families not eligible for GPD services.
- Perform intake including required documentation for admission, referral needs, program details and expectations, conduct written assessment on all potential Veterans and their families within 72 hours of contact including screening for serious personal safety and mental health issues and develop a housing plan with the participant within one week with specific short and long term housing goals with objectives and update case plans as needed.
- Schedule regular meetings with participants to review and update goals. Conduct home visits as appropriate.
- Provide participants linkages to community services.
- Provide services as needed, e.g. court appointments, landlord/tenant conflict resolution, conduct life skills and budgeting classes and completes required documentation on all educational groups.
- Provide financial assistance in accordance with GPD program policies and procedures.
- Coordinate case management efforts with VA, SSVF staff and other Veteran Service Providers to meet family needs.
- Develop an exit discharge plan addressing permanent housing, economic and emotional stability as well as self-determination and provide a “warm hand off” to partnering staff/agency.
- Maintain comprehensive and detailed case files in HMIS on all participants that includes the assessment, case plan, documentation of progress, challenges, accomplishments, records of financial assistance provided to participants, discharge plan and documentation of all follow up contact.
- Ensure client confidentiality in accordance with established procedures and regulations.
- Submit monthly summary of service statistics to the GPD Coordinator. Complete other reports as requested.
- Participate in quality assurance activities evaluating program effectiveness and outcomes including reporting effectiveness of serviced delivery by using consumer and community resource feedback.
- Participate in training and workshops as directed by the GPD Program Manager and/or DOVS.
- Maintain positive work atmosphere by behaving and communicating in a manner that fosters good relationships with clients, co-workers and supervisors. This includes, but is not limited to: resolution of conflicts in a professional manner, courteous treatment of staff, visitors and clients; respect of others’ property and person, and professional and appropriate communication to and about clients, co-workers and supervisors
- Attend agency and community meetings, CoC Subcommittee on Ending Veteran homelessness meetings, Participate in Case Conferencing and maintain working relationships with community agencies (veteran assisting organizations including Veterans Affairs, VACs, Veteran Benefits representatives, etc.) to provide comprehensive services for participants and to stay abreast of current trends and resources available.

Physical Demands/ Work Environment:
This position is required to do light to moderate physical work. Must be able to lift 30 pounds. In order to successfully perform the essential functions of this position, the individual is regularly required to remain stationary for some periods of time, use hands and fingers, communicate, move, occasionally required to position self to stoop, kneel or crouch, frequently required to reach with and use arms and hands as required in using standard office equipment and a PC. Specific vision abilities by this job include close vision, distance vision and ability to focus. The work environment for this position includes an office environment with a low noise level.
