Summary of Position

The Salvation Army Northern Division is hiring a Case Manager to provide direct services of HeatShare and Emergency Social Services in a professional manner to clients; provide case management services (assessment and planning).  To ensure that services are responsive to the needs of the community that particular office serves, and to be an active partner in the community.  This position works in an independent manner with little direct supervision of the Social Services Coordinator III. 

 

Pathway of Hope is an approach to providing targeted services to families with a desire to take action to break the cycle of crisis and enable a path out of intergenerational poverty

 

This is a regular full-time position with a work week expectation of 40 hours per week.

 

Expected pay for this position is $20.50 dollars per hour.

 

The main work location will be at 7380 Afton Rd. Saint Paul, MN 55125

 

Qualifications

  • Bachelor’s degree in Social Work or a related field. (If degree is in Social Work, must be licensed by the Minnesota Board of Social Work.)
  • Experience working in a social service setting required.
  • Understanding of the delivery of Social Services.
  • Ability to work well with clients of diverse cultural and socioeconomic groups; respect confidentiality; able to work within ethical and legal parameters of social services.
  • Ability to accept supervision and work with a team of co-workers.
  • Ability to organize work, set priorities, meet deadlines, and complete tasks with a minimum of direction.
  • Ability to use Salvation Army approved word processing, spreadsheet, data base management, and graphics/desktop publishing software packages.
  • Valid Driver's License. (Must meet the guidelines provided by the State of residence and the automobile insurance provider as pertaining to corps’ passenger vans/buses.)
  • Must pass The Salvation Army background checks and meet the MVR driver’s qualifications.
  • Must complete The Salvation Army Caseworker Certification Program within twelve (12) months of hire.
  • Willingness to support the philosophy, goals, objectives, and mission of The Salvation Army.
  • It must be understood by the incumbent that the provisions of Safe From Harm will apply upon commencement of employment and also understood that the required Safe From Harm training be satisfactorily completed within the first thirty (30) days of employment.

Essential Duties and Responsibilities

 

This job description is not all inclusive.  It is intended to identify the essential duties of the position.  Other duties, responsibilities and tasks may be assigned by the Social Services Coordinator III.

 

  • Maintain positive work atmosphere by behaving and communicating in a manner that fosters and maintains positive relationships with co-workers and supervisors. This should include, but not necessarily be limited to such actions: resolution of conflicts in a positive manner; courteous treatment of staff, clients and visitors; respect of others’ property and person, professional and appropriate communication to and about co-workers, supervisors and subordinates.
  • Support the Mission of The Salvation Army by treating every donor, contact and colleague appropriately and without discrimination of any kind.
  • Adhere to the guiding principles of The Salvation Army's social services ministries as outlined in Faith In Action with an emphasis on intentional outreach in the area of pastoral care to those we serve.
  • Conduct client interviews and case management. Determine eligibility for programs, assist in application, provide options and make referrals.
  • Provide crisis intervention assistance when needed.
  • Make home visits to clients when appropriate.
  • Keep accurate case notes of phone calls, interviews, etc.
  • Assist clients with direct assistance (foodshelf, HeatShare, rental assistance, vouchers, etc.).
  • Maintain required statistics (i.e. TSA stats, client files, United Way, etc.).
  • Participate in office and department meetings, committees, annual retreat, and other meetings.
  • Participate in community meetings and networking.
  • Facilitate educational/support groups and complete statistics.
  • Assist in planning and implementation of seasonal activities as requested.
  • Participate in the development of office goals together with the Social Services Coordinator III; participate in development of departmental and personal goals.
  • Develop working relationship with Corps and officers.
  • Conduct Pathway of Hope recruitment and case management. Responsible for the provision of a full range of case management interventions including engagement of new clients, completion of comprehensive assessments and action plans, referrals to services designed to address identified goals and development of plans to address transition needs and ongoing documentation of progress
  • Assist with other department functions (i.e. disaster, special events, committees, etc.).
  • Participate in monthly supervision meetings with the Social Services Coordinator.
  • Data entry, computer updating as requested.
  • Attend educational opportunities when appropriate.
  • Other duties specific to the office location and as assigned. Actual duties performed vary widely according to the services offered by the office a staff person is assigned to.
