Qualifications: 5 years experience in Long-term Care Management and Licensed Registered Nurse As a Corporate Case Manager in a Grace Management Services facility you must be able to perform the following essential functions as demonstrated by the ability to:
Essential Job Functions:
Serve as a liaison between hospitals and nursing home facilities by dealing directly with Social Services, Discharge Planners, and Case Managers.
Prompt response to referrals/inquiries and performs diligent follow up.
Must be able to evaluate potential residents needs on an individual basis to determine what level of care is needed.
Verify insurance information on potential residents, and complete any pre-certifications (related to patient care) that potential residents may need upon admission.
Work with Admissions, Social Services and Nursing departments at skilled nursing facilities to ensure smooth transitions for our residents from hospital care to long term care facilities.
Documents information correctly and utilizes technology to improve efficiency.
Promotes a positive image of Sprenger Health Care Systems at all times.
Seek out opportunities for business and census development with physicians/hospitals.
Informs Director of Business Development of changes in the marketplace, hospital systems and any challenges faced during the conversion of referral to admission.
Schedule CEU events in partnership with area hospitals and follow-up with Regional Marketing Director to coordinate event.
Research opportunities to partner with hospital organizations for staff training.
Possess awareness of cost saving opportunities during assessment process.
Promote all services Sprenger Health Care Systems offers.
Must be able to speak in public and in front of large groups.
Clear written and verbal communication skills.
Proficiency in Word and Excel.
Good time management skills.
Works well in a team environment.
Ability to maintain confidential information.
Perform other related duties as directed assigned.
