Spring River Mental Health & Wellness, Inc. is a community mental health center located in Cherokee County, KS with offices located in Riverton and Columbus. This position will be based out of our Columbus office location.

Position Summary: Case Managers provide strengths-based direct care services to youth with a severe emotional disturbance (SED). The majority of the Case Manager's time will be spent providing services to youth in the community setting such as school, home or other other community settings.

Work Schedule: Case Managers work Monday - Friday. While a case managers schedule can vary and is flexible to meet client needs, a typical work day is 8am - 5pm.

Hourly Pay: Minimum hourly wage starts at $19.71/hour and is commensurate based on the individuals level of education/degree, specialized training and years of relevant experience.

Primary Duties and Responsibilities:

1. Provide goal directed supports and solution-focused interventions intended to help the child make progress and achieve treatment goals outlined on the individualized treatment plan.

2. Ability to work professionally and in conjunction with other mental health center staff and community stakeholders.

3. To assure that referrals for case management services are acted upon timely, typically within 3 business days of referral.

4. To assure that clients will be assessed via the strengths/needs assessment tool and services provided based on level of care.

5. To assure timely completion, documentation, and/or submission of service notes, assessments, and data reporting within agency required timeline.

6. To meet direct service expectations.

7. To complete training requirements as necessary for provision of case management and additional Rehabilitation Services, as determined by supervisor and KDADS.

8. To provide crisis intervention according to policy.

9. Ability to provide additional rehabilitation services such a psychosocial groups, as needed.

Community Psychiatric Support and Treatment (CPST) definition: Goal directed supports and solution-focused interventions intended to achieve identified goals or objectives as set forth in the consumer’s individualized treatment plan. CPST is a face-to-face intervention with the client present.

Qualifications:

  • Minimum of 21 years of age.
  • Bachelor's Degree in a related field strongly preferred. Candidates that are equivalently qualified through a combination of related work experience and education, with one year of experience substituting for one year of education will be considered.
  • Minimum of one (1) year of related experience preferred.
  • Meet specifications outlined by KDADS licensing Policy. Completion of background checks, drug screening process, possess a valid drivers license and successfully complete motor vehicle record check.
  • Excellent work ethic and reliability is a must.

Knowledge, Skills, Abilities:

  • Familiarity with mental health conditions in children and ability to relate a strengths based approach in your work.
  • Effective interpersonal skills and the ability to communicate both orally and in writing.
  • Computer proficiency to use electronic health record and other information systems.
  • Ability to establish therapeutic relationships with children and families and develop effective working relationships with staff and community agencies.
