JOB SUMMARY: The Director of Social Services is responsible for planning, organizing, staffing, directing, coordinating, reporting, budgeting, and physical management of the social service department's employees and equipment in order to implement group and one-to-one psycho-social programming as well as meet with interdisciplinary team members. The Director of Social Services reports directly to the Administrator.
JOB REQUIREMENTS:
1. Must be physically and mentally capable of performing routine job duties.
2. An understanding of the psycho-social dynamic of the geriatric population and the ability to empathize and provide guidance to them.
3. Ability to relate well to and communicate with residents, family members, and staff.
4. Good administrative, managerial, organizational, planning, and interpersonal skills.
5. Must have compassion, tolerance, and understanding for the elderly.
6. Knowledge of ODH regulations relevant to social services.
MAIN DUTIES:
A. Support the facility's philosophy of care and strive to achieve its goals and objectives.
K. Schedule group and one-to-one sessions to accommodate all appropriate residents.
L. Review monthly progress notes, quarterly assessments, and social histories and countersign them.
N. Complete the Psycho-Social Section of the MDS form upon admission, quarterly, annually and/or in cases of significant change and coordinate suitable programming for residents identified with psycho-social needs.
O. Track discharge and readmission of residents to determine the need for reassessment and inform the staff to follow up.
P. Identify and communicate with the Psycho-Social staff regarding any needed change in goals, program schedules, etc.
Q. Review Psycho-Social care plans for their resident appropriateness.
R. Attend admission, Care Plan, Medicare, Department Head, and Performance Improvement meetings and others as deemed appropriate.
S. Assist in the determination of appropriate room changes and speak to residents and family members when necessary.
