The Case Manager for our Outpatient Program is responsible for
clinical and medical case management for our ABH clients.

Duties & Responsibilities
Obtain initial insurance authorizations.
Obtains extensions for insurance authorizations.
Check daily attendance and maintain admission log and distribute
weekly census report.
Facilitate groups.
Document all collateral contacts and services in our EMR system.
Track and collect fees, when applicable.
Meet with clients to develop and review treatment plans.
Link clients to appropriate services, such as primary care,
transportation, and psychiatric.
When discharge is planned, arrange for aftercare appointments.
Phone or letter outreach for clients who drop out.
Participate in team meetings and program supervision.
Markets the program.
Participate in ongoing development of curriculum for ABH.
Provide one-on-one counseling.
Complete discharge summaries/ Disenrollment MIS (EIM/ESM).
Answer phone inquiries about the program from prospective client
and referral sources.
All other duties as requested.
Experience and Skills:
Duties & Responsibilities
Bachelor's degree, preferred.
CAC/CADAC, preferred.
Demonstrated effectiveness in working with and engaging an
addicted population.
A minimum of two years experience in substance dependence and/or
mental health field.
Strong organizational skills.
Ability to function independently with minimal supervision.
Solid understanding of insurance procedures and payor system.
Knowledge of community services - especially SSTAR services.
