The Case Manager at St. Ann’s Home exemplifies the vision, the mission, and the vibrancy of the organization and leads the delivery of exceptional care to our residents. Teamwork and collaboration are instrumental to the success of our communities, and in this role, you will work with the internal leadership team, including our Director of Nursing, and our external partners to oversee and guarantee that our resident's needs are not only met, but delivered with vibrancy. 

Essential Functions:

  • Day to Day operations: The primary purpose of your job position is to plan, organize, develop, coordinate, and advocate for our resident’s care in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Director of Nursing/Administrator, to assure that the medically related emotional and social needs of the resident are met and maintained on an individual basis.
  • Duties/Responsibilities: In this role, you will assist the residents in adjusting to the facility and promote a positive environment for the continuity of relationships with family and community. Learns the resident’s desires for a full life and works to make them happen. Assist the residents and families to utilize the community resources when not provided directly by the facility, ensuring that residents and families feel sufficiently equipped and supported. Assist residents and families with the Medicaid application process and direct them to community support agents. Participate in discharge planning, development and implementation of social care plans, and resident assessments. Performs verification of utilization criteria reviews. Oversee Behavior Management Program and tracking of psychotropic medication utilization. Review departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken. Assess and evaluate each resident’s psychosocial needs and develop goals for providing the necessary service and take part in the admission process by conducting and coordinating admission and discharge assessments with residents and their family.
  • Administration: Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. This includes ensuring the social services components of the MDS are completed and signed on a timely basis. Participate in daily nursing report along with daily electronic documentation. Assist the Quality Assurance Committee in developing and implementing an appropriate plan of action to correct identified deficiencies. Participate in facility surveys (inspections) made by authorized government agencies as necessary, or as may be directed. Keep abreast of current federal and state regulations and professional standards and make recommendations on changes in policies and procedures to the Administrator. Assist Director of Nursing/Administrator in abuse/neglect/misappropriation investigations. Consult with/support Assisted Living Manage with any psychosocial issues that may arise.
  • Communication: Facilitates multi-disciplinary team meetings, including physicians, nurses, therapists, and ancillary departments. Organize and lead Care Planning Conferences, Resident Council and Family Council. Incorporate the Social Services goals into the resident’s plan of care. Provide residents with information concerning resident rights, living wills, etc., and explain as necessary. Meet with administration, medical and nursing staff, and other related departments in planning social service programs and activities. Serve on, participate in, and attend various committees of the facility as required, and as appointed by the Administrator.
  • Teamwork: Will assume responsibility for participation in on-call rotation for leadership. Assist with Admissions and discharges. Oversee assigned staff related to development, performance, and in-servicing/continuing education.
  • Unparalleled Service: Uphold the highest standards of self and team of customer service to residents. Assist team as needed, to exceed resident’s expectations.
  • Flexibility: The ability to shift gears and implement new topics and processes quickly to address emerging resident needs. The work schedule may require conformity to resident needs and outcomes.
  • Other duties as assigned, to meet standards and organizational goals.

Education and Experience Requirements:

  • Must possess, as a minimum, a Licensed Practical Nursing Degree from an accredited college or university
  • Must possess a current, unencumbered, active license to practice as an LPN or RN in this state.
  • Current CPR certification also required
  • Must not pose a direct threat to the health or safety of other individuals in the workplace
  • Previous long term care working experience
  • Excellent verbal communication skills with the ability to interact with a diverse group of associates, residents and family members.
  • Ability to apply reason, logic, and advanced problem-solving skills to resolve complex and/or sensitive issues.
  • Excellent written communication skills including proper grammar and professional writing.
  • Ability to read at a level to understand care notes and written directives of medical practitioners.
  • Strong computer and keyboarding skills with proficiency in Microsoft Word and Excel and the ability to quickly master new applications.

Physical Environment:

  • Must be able to move intermittently throughout the workday.
  • Must be able to speak and write the English language in an understandable manner.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to see, hear or use prosthetics that will enable these senses to function adequately to ensure that this position's requirements can be fully met.
  • Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within St. Ann’s Home.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Job Type: Full-time

Pay: $65,000.00 - $75,000.00 per year
