POSITION SUMMARY:

The Community Crisis worker is responsible for serving within the hospital as part of the community-based team that offers crisis intervention to family members of patients who have experienced violence, provide advocacy and accompaniment, develop case plans, provide referrals to other service supports, and intervention with third party institutions, as necessary. Referrals are made to the department from the hospital, other hospital emergency departments, the outreach team, community agencies and direct referrals from staff physicians. Screening is conducted to ensure that an individual is appropriate for the services offered. All individuals presenting to the hospital’s ED and those referred from other hospitals or facilities are evaluated in a face-to-face interview to determine the intensity and severity of the presenting symptoms so that appropriate safety measures are recommended.

PRINCIPLE RESPONSIBILITIES:

  • Collects demographic data including the presenting problem.
  • Provides case management, accompaniment, and referrals to family members of victims who have experienced violence, along with managing crisis situations and providing appropriate interventions which includes and is not limited to intervening with employers, landlords, or academic institutions.
  • Provides community-based outreach in assigned communities and other community settings as needed.
  • Collaborates with victims and their families to develop case plans.
  • Provides off-site educational workshops and services, along with establishing and facilitating support groups.
  • Actively contributes to and participates in supervision, staff meetings, in-services, and mental health coalition meetings.  
  • Documents all interactions and services provided in the client's case file and manages files according to regulatory requirements.
  • Develops and maintains a list of resources for referral purposes.
  • Participates in program planning and evaluation activities.  
  • Accountable for monitoring, documenting, and tracking program and client data and assuring compliance with grant-related or hospital-related documentation requests made by supervisor.
  • Attends trainings and workshops assigned to provide culturally appropriated and quality services to the population served.
  • Performs other activities as needed and as program evolves.

 

ADDITIONAL RESPONSIBILITIES:

  • Bachelor’s degree in social services field or related fields or a minimum of three years of experience working on crisis intervention and case management
  • Collaborates with team members by sharing relevant information, demonstrating initiative, and supporting team decisions as well as the work life of the department.
  • Ability to work both independently and as a team member
  • Performs both essential and marginal job functions safely as identified by Saint Anthony Hospital and the respective department while adhering to the Hospital’s confidentiality norms and standards.
  • Participates in performance activities including problem identification, data collection, solution selection, implementation, and evaluation.
  • Maintains professional knowledge base through literature review, seminars, workshops, and meetings throughout the calendar year. Participates in professional association activities to assist in meeting CE requirements.
  • Bilingual English/Spanish (Required)
  • Ability to work evening and weekends
