- Data Review/Analysis: This will have been satisfactorily performed when
- Reports from AS400 are run in order to identify patient population for data extraction.
- Required data related to the identified core measure is extracted from patient record and the core measure tool is accurately populated with required data.
- Data from the previous quarter is validated by the end of each quarter for completeness and accuracy prior to the deadline for abstraction to CMC.
- Compliance and Regulatory: This will have been satisfactorily performed when
- Reports from the vendor tool are run and populated as needed and at the request of administration as evidence by an accurately and up to date populated Hospital report-card.
- Excel variance report for each core measure assigned for the month is populated by the end of the month.
- CDAC charts submitted to Medicare pass audit at 95% . Keeps abreast and validates the Specification Manual Updates through downloads.
- Nursing core measure accountability report-card is populated monthly.
- Consultation and Partnerships: This will have been satisfactorily performed when
- Serves as a resource to team members on issues related to core measures.
- Audio Conferences on Specification Manual Updates are attended quarterly.
- Attendance at Core Measure Team Meetings is at 90% and input is provides as needed.
- Assistance is provided to team leaders of Core Measure Teams with education of identified staff.
- Assistance with the development of procedures and tools to meet changing standards is provided.
- Other Duties as Assigned: This will have been satisfactorily performed when
- Assistance is provided for projects such as Patient safety week as needed.
- Other duties as assigned are completed.
