Job Summary:
The Performance Improvement Specialist plays a pivotal role in enhancing patient outcomes, operational efficiency, and overall quality of care. This individual will conduct in-depth analyses of clinical operations, identify improvement opportunities, and lead initiatives to optimize processes.
Responsibilities:
- Perform in-depth analysis of clinical operation trends and patterns to identify areas for improvement.
- Conduct comprehensive performance improvement projects, including data collection, analysis, and reporting.
- Develop and implement effective action plans to address identified performance gaps.
- Collaborate with cross-functional teams to drive process optimization and change management.
- Develop and deliver training programs on quality improvement methodologies and tools.
- Foster a culture of continuous improvement through effective communication and engagement.
- Utilize data analytics and performance metrics to monitor progress and measure outcomes.
- Participate in rapid cycle improvement initiatives to drive timely and sustainable changes.
- Monitor external reporting requirements for CMS, TJC, GWTG, and other agencies and facilitates implementation, validation and enhance practices
- Demonstrate a comprehensive understanding of quality reporting programs, core measures, eCQMs, and HEDIS.
- Facilitate the submission of Leapfrog Hospital Survey and Leapfrog Safety Grade.
- Develop and implement strategies to improve performance on Leapfrog metrics.
- Participate in quality improvement initiatives and regulatory compliance activities.
- Stay current with industry trends and best practices in performance improvement.
Requirements:
- Bachelor's degree in healthcare administration, nursing, or a related field required.
- Minimum of 3-5 years of experience in performance improvement or quality management.
- Knowledge of healthcare quality Improvement methodology, group facilitation, and project management
- Knowledge base includes TJC standards interpretation, CMS and NYSDOH regulations, CMS quality reporting programs, core measures, eCQMs, and HEDIS.
- Strong analytical and problem-solving skills with a data-driven approach.
- Proficiency in data analysis and statistical software (e.g., Microsoft, Google, Tableau).
- Excellent communication and interpersonal skills.
