What you will do:
Empower people to live their best life by providing inclusive and family centered supports
- Provide home and community services to assigned case load (9-11 parents and their respective children)
- Develop and implement lesson and program plans for children of PLT families
- Assist with the development of the Individual Family Service Plan for assigned caseload
- Assist with appropriate parent-child outings and act as positive role-model
- Coordinate and track medical and dental appointments of PLT children
- Respond to parenting needs of families enrolled in the PLT program
- Assist with medical decisions in emergency situations
- Assist parents with personal safety decision making in crisis and non-crisis situations utilizing person centered philosophy
- Participate in support plan meetings, and other conferences regarding client needs
- Attend necessary educational meetings regarding school aged PLT children
- Maintain assigned clients’ records and reports within identified guidelines and timelines
- Maintain food and program supplies as directed by Program Coordinator
- Develop and maintain relationships with other service providers to identify a wide array of resources for families

What we expect of you:

- Required Education:
- High School Diploma or equivalent required
- Associates degree in Child Development or related field preferred
- Minimum Years of Experience:
- Two or more years of experience working with parents with IDD/mental health disorders in a social service environment
- Other Requirements:
- Agency required training online and in the classroom
- Valid Driver’s License
- Valid Auto Insurance
- CPR/First Aid
- Covid Vaccinated

Salary : $32,400 - $41,000
