RN Team Coordinator is an office based position thatprovides and directs the provision of home care, based on agency policies and procedures, through the competent application of the nursing process. Conducts field visits when warranted. Collaborates closely with disciplines to ensure patient needs are met in accordance with plan of care. Provides oversight to decrease any disruption in home care services. Maintains clinical records in accordance with New York State and federal regulations, and Home Care policy. Supports Hospital's mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.
JOB DUTIES / RESPONSIBILITIES
1. Ensures Home Care services are provided in accordance with the plan of treatment signed by the physician.
2. Collaborates with the disciplinary team, Physician and family to develop a plan of care and evaluates and makes necessary revisions to the patient's plan of care when there are changes in patient condition. Addresses any problems that may arise regarding Managed care and physician orders. Additional with interdisciplinary team Reviews clinical notes, follows up with managed care to ensure authorization is secured. Ensures summaries are well documented and submitted timely as per policies and practices.
3. Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to the patient's care and safety. Makes referrals and secures MD order for other disciplines as indicated by the patient's needs, or documents rationale for not doing so.
4. Reports to the physician and/or other appropriate health professionals as often as needed, or upon changes in patient's condition. Reconciles medication profile with RN and physician as needed.
5. Reviews and coordinates the total plan of care and maintains continuity of patient care by collaborating with other health professionals assigned to the same patient. Initiates and attends patient care conferences as needed for complex and/or multi-disciplinary patients, when needed.
6. Maintains collaborative relationship with the Team Supervisor to ensure adequate coordination of care in accordance with Home Care policy and regulations.
7. Ensures home health aide supervision occurs every two weeks if required. Maintains open communication and ensures plan of care is revised as needed. Communicates the plan of care changes to the aide and vendor agency as often as necessary.
8. Ensures proper discharge planning and notification in collaboration with staff, therapist, and physician. Completes/ensures discharge summary is submitted.
9. Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate and timely documentation in accordance with program policy.
10. Responsible for following all policies and procedures of the agency regarding service delivery, documentation and care coordination. During supervisory visits, exhibits full compliance and verbalizes knowledge of agency policies including, but not limited to, patient education, infection control, and management of hazardous wastes and age-specific behaviors of patients.
11. Demonstrates competency in all skills required for the agency, including expertise in pediatrics.
12. Participates in on-call rotation.
13. Performs record review to ensure quality documents and compliance with policies and regulations.
14. Reviews and ensures MD orders are generated daily and in accordance with policies and procedures.
15. Reviews Telehealth alerts on a daily basis, triage calls and applies appropriate interventions.
16. Performs other duties as assigned.
Formal education required:
Associates degree in nursing; B.S.N. preferred.
Holds a valid registered nursing license to practice in New York State.
Previous experience required:
Minimum of one year of nursing experience in pediatrics required
Home care experience preferred.
