Case Manager

Who We Are

St. Mary's Home for Boys offers residential and day treatment services to at-risk clients between the ages of 13 and 17 who have emotional and behaviorally challenges. These clients are referred to the agency by the Department of Human Services (DHS) or Oregon Youth Authority (OYA). The individualized program provides each client with a structured regimen to ensure a successful transition to public school, the community, and an appropriate living environment. We have been providing services to youth since 1889.

What you will do:

• Provides individual, group & family counseling to a caseload of 9-10 clients.
• Responsible for developing Comprehensive Case Assessment with Service Plan in collaboration with the Collaborative Problem Solving (CPS) model.
• Create treatment goals and transition planning with clients and their family or identified aftercare resources.
• Comfortable providing treatment to clients with sexually harmful behavior.
• Works harmoniously with referral source in developing and implementing aftercare programs involving home, community, and school.
• Work in collaboration with a clinical team to provide comprehensive treatment reviews.
• Help coach residential staff on therapeutic treatment interventions.
• Authorizes all Isolation and Seclusion Room Placements for clients in the program on a rotating schedule.

Educational and Experience Requirements:

• MA/MS in Psychology, Sociology, Counseling, Social Work and/or closely related field.
• One-year experience in the care & treatment of children or young adults.
• Experience in a residential treatment facility or similar setting with cognitive behavioral programming, case management experience and Trauma Informed Treatment skills preferred.
• Collaborative Problem Solving experience.
• Knowledge of Collaborative Problem Solving model is preferred.
• Strong, motivational communication skills.
• Excellent writing & organization skills with attention to details.

Additional Information:

• Starting range:$57,000 to $63,000 (depending on experience).
• Core schedule: Monday–Friday; 9 am–5 pm; Hours may vary to meet agency needs.
