ABOUT THIS ROLE:
Provide case management to the residents in Emergency Housing/Shelter of St. Matthew’s House. The Case Manager will assist the resident in achieving self-sufficiency through advocacy; setting goals, accessing community resources and support services. Case manager will work one-on-one with residents to develop goals and evaluate progress.
St. Matthew’s House is a faith based 501(c)(3) charitable organization that exists to transform lives in a spiritual environment that is both compassionate and disciplined.
ESSENTIAL TASKS:
- Collaborate with residents in the development of their initial individual case plan.
- Assist residents in completing necessary paperwork for goal achievement, referral services, and supplemental documentation as needed.
- Establishes goals specifically in the following categories1) obtaining ID 2) job attainment 3) education or training 4) treating addictions, MH treatment plan and utilizing counseling opportunities 5) growing spiritually through church attendance and daily devotions 6) financial stability through banking and budgeting 7) compliance with legal consequences. 8) Transition to sustainable affordable housing
- Prepares and maintains objective case notes and/or comments, using the SOAP method, on residents during the week; ensures the accuracy and completeness of same.
- Maintains a weekly calendar, in the Client Management System, of all scheduled Case Management appointments with residents
- Meets weekly with residents to evaluate their progress and establish stability; prepares accurate, objective and up-to-date case notes documenting interactions.
- Refers residents to agencies based on resident’s need and in accordance with their case plan.
- Works alongside volunteers in providing life skills classes as resident needs are uncovered.
- Checks multiple times per week to ensure residents’ living area are properly maintained and in compliance with Shelter guidelines.
- Provides relief for staff members during staff breaks
- Performs random drug screens on residents as needed.
- Ensures that the facility is maintained in a safe and secure manner.
- Completes discharge of residents, as needed, with accurate information.
- Meets with immediate supervisor weekly to provide updates on the status of resident’s progress.
- Utilizes staff resources to determine available options for residents with unique needs
- Treats all residents with respect and exercises appropriate boundaries with residents.
- Adheres to the Core Values of St. Matthew’s House in the work place.
- Ensures compliance of residents with the guidelines in the Shelter Handbook
- Any ancillary tasks assigned by the Assistant Manager or Program Director.
- Presents the Gospel message to residents when the opportunity is available.
- Pray with residents and staff as opportunities are available
Requirements
Education: High School Diploma required or equivalent; Certification in Addiction preferred.
Experience: A minimum of 1 year of experience working with individuals who are homeless or suffering from addiction; Ministerial counseling preferred.
- Must be able to stand for a minimum of 4 hours at a time and lift at least 25 pounds.
- Must be able to bend or kneel to the ground.
