What are the responsibilities and job description for the Nurse Coordinator position at ST. VINCENT\?
A Registered Nurse who assists in providing nursing services, education to staff and clients and is a vital member of the Adult Homes Supported Housing team. The Registered Nurse is part of the interdisciplinary team and serves as the expert in for the assessment, identification of needs and coordination of care and services to facilitate the patient's progression through the continuum of care in a timely and cost-effective manner. Care management including care planning, coordination, and assessment of health, environmental and psychosocial status members on an ongoing basis.
Additional Requirements
- Minimum of two years of experience in specialty or area of practice, e.g. acute care, ambulatory care, long term care, behavioral health.
- Good written and verbal communication skills.
- Good interpersonal skills
- Ability to work as part of a team
- Ability to work in a fast paced environment.
- Some lifting of medical equipment and supplies. Prolong hours of sitting and/or standing. Travel to and from clients' home and walking up and down stairs. Ability to travel to clients home and conduct nursing and behavioral clinical visits. Able to carry medications to clients home and back to office.
Education Requirements: Associate or Bachelor degree required; NYS Professional Licensed required (RN).
Valid driver's license in good standing
Proficient in Microsoft Office Suite (Outlook, WORD, Excel, PowerPoint) and database navigation required.

Pay: $85-90K per year
Salary : $85,000 - $90,000
