  • Part Time (Per Diem) - Flexible

Staff Mates Home Care is a family owned home health care agency with over 40 years of history providing quality care to families in the Willimantic and Hebron Areas.

  • RN position entails home visit and case management responsibilities include case management and home visits to agency clients.
  • Conducts start of care visits developing patient centered plan of care.
  • Visit activities also include Recertification, Resumption of Care, and Skilled Nursing Visits.
  • Participates in chart review and performance improvement program.
