Level/Salary Range: $52,000-$62,000 Annual Salary
Position Type: Full Time 40 hours a week or possibly part time

POSITION SUMMARY:
Provide nursing services to adults with intellectual and developmental disabilities (IDD) enrolled in Starpoint programs including but not limited to: Physical Assessments, treatment planning, timely documentation, coordination of medical treatments, medication administration, monitoring of health status of individuals,
CORE RESPONSIBILITIES:
· Communicate with those we serve, caregivers, families, teams, and caseworkers daily.
- Attend meetings and appointments as needed.
- Assess, report, train, educate, and triage. We respond to the needs, concerns, and requests of those we serve.
- Write comprehensive care plans and maintain electronic medical records.
- Monitor client appointments and follow-ups. Process medication orders and treatments.
- Work with medical providers and staff to coordinate continuing care.
· Communicate and coordinate with the Chaffee Program Manager regarding the provision of Starpoint’s medical services and supports.
· Oversee non-skilled patient care provided by Direct Support Professionals to ensure our patients are healthy and safe.
· Assure quality medical oversight for all individuals we serve.
· Assure quality medical procedures, physician’s orders, and medication management are followed.
· Oversee, and instruct as needed, initial medication management and refresher courses, G-tube feeding procedures, and other specialized medical procedures, as needed.
· Develop, implement, train, and follow up on any individual specialized medical needs/protocols for those we serve.
· Receive or obtain telephone or written MD orders.
· Follow-up on any extensive medical care needed by those we serve.
· Be available for a phone consultation (if possible).
· Assure that medical care seems appropriate and reevaluate if necessary.
· Assure compliance with Department of Human Services and Department of Health rules and regulations.
· Be available for all Department of Health and Division inspections.
· Strictly adhere to all policies/procedures relating to confidentiality.
· Transport consumers in own or company vehicles as necessary and/or required.
· Be familiar with and abide by all agency policies and procedures.

SKILLS & EXPERIENCE (These are the education and experience requirements.)
Current, valid State of Colorado Registered Nurse license. Current CPR certification. Prefer experience in a clinical setting (nursing home, hospital, or group home.)

· Must be familiar and proficient in the use of computers (both desktop and laptop)
· Must have the ability to use Microsoft Office including Microsoft Word, Microsoft Excel, Microsoft Outlook, and other Microsoft Software
· Must have the ability to understand and use the internet for purposes of searching for resources, researching providers, and more.
· Use computer software to document patient care.
· Must be able to use an agency phone, set up an office voicemail, and listen to and respond to messages promptly.
ESSENTIAL FUNCTIONS:
Might be required to carry 50 pounds; to push/pull 100 pounds; lift heavy objects; stand 4 hours per day; walk 4 hours per day, etc.
