Program:
Transition Age Youth Program - Alameda
Starting pay: $24 per hour
JOB SUMMARY: This position has primary responsibility for case management services to clients and their families referred through Stars’ Transition Age Youth (TAY) Program (TAY).
MINIMUM QUALIFICATIONS
Education and Experience
- 4 years of experience in a social service field OR
- Associate Degree plus 2 years of experience in a social service field OR
- Bachelor’s degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service field
License or Certification
Valid California Driver's License.

DUTIES AND RESPONSIBILITIES
Essential Duties
1. Work within the program model’s framework, including appropriate Evidence-Based Practices, to provide services to clients and their families referred through Behavioral Health Care Services (BHCS).
2. Provides services at the TAY Center, in the community, and in TAY’s homes. Uses personal vehicle to drive within the community to provide services and to transport TAY and/or TAY’s family members in personal vehicle.
3. Provides case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
4. Meets or exceeds expected billable hours.
5. Participates in meetings, such as Treatment Team Meetings, to ensure clients’ needs are met and recommendations are followed through.
6. Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established by Stars.
Other Responsibilities
1. Assists the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
2. Monitors the clients’ progress toward goals outlined in their treatment plans. Informs primary therapist of progress.
3. Develops resource directories related to housing and social support resources.
4. Assists clients with securing housing.
5. Assumes role of Sub-Payee for assigned clients.
6. Develops and maintains relationships with external partners, including DOR, BHCS, Conservators, other CBOs, etc.
7. Co-facilitates groups with therapists and/or Peer Mentors.
8. Has the ability and willingness to contribute to agency quality efforts.
