Teammates - Wraparound Fresno
JOB SUMMARY:
The position has primary responsibility for working with parents/caregivers to provide support, education, and advocacy to successfully navigate the system in support of their child(ren) and family.
MINIMUM QUALIFICATIONS
Education
High School diploma or equivalent
Experience
Personal experience as a consumer, foster parent, or parent of a child in the child welfare, probation, or mental health systems.
License or Certification
Valid California Driver's License preferred
Specialized Skills
Bilingual in Spanish language preferred but not required.
DUTIES AND RESPONSIBILITIES
Conducts a strengths interview with family members; tells own story to engage families. Identifies family strengths for problem solving and assists family in identifying specific underlying needs. Advocates for and works with family to represent their best interests. Articulates family perceptions for other team members. Educates family members on available resources and how to access them. Links family members to identified resources, including housing, food, education/vocation, recreation, spiritual, and social, as deemed appropriate in partnership with the treatment team. Availability by phone and in person to assist in crisis intervention and provide support and assistance in problem resolution. Helps re-evaluate parental needs and goals following a crisis. Provides and documents collateral, case management and linkage services. Documents services related to specific domains from the parent perspective. Participates in various meetings on an as-needed basis, including Child and Family Team (CFT) meetings and Family Therapy sessions to support the team with meeting the family’s needs.
