The Department of Economic Security, Division of Developmental Disabilities (DDD) is seeking an experienced and highly motivated individual to join our team as a Case Manager Lead – Behavioral Health. This position serves persons with developmental disabilities (DD) and Mental Illness (MI) by supporting them and their families to lead the healthiest, most-sufficient and productive lives within their capabilities
- Completes progress notes, facilitates, writes and reviews Individual Support Plans (ISPs), required reports, letters and other correspondence as needed
- Conducts field visits per agency policy requirements at various locations; family homes, group home and other residential placements, psychiatric hospitals, schools, day programs, work programs, hospitals, etc., to develop relationships with child/adult and families
Attends discharge staffing at psychiatric facilities, petitions and attends hearings for Mental Health Court Ordered Treatment, attends Child Family Teams (CFT) for children in residential group home placement - Works closely with various mental health agencies and the local psychiatric facilities in managing member services and assisting in ensuring a prompt discharge when member has been involved in an in-patient stay
- Monitors appropriateness and safety of member’s placement, assesses mental health status, strengths and needs; extensive problem solving and conflict resolution within member teams; works in conjunction with other team members and the supervisor to determine appropriate case plan goals
- Knowledge of Health Insurance Portability and Accountability Act (HIPAA)
- Knowledge of Federal and State statues and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities
- Knowledge of medical and behavioral health issues and terminology
- Knowledge of methods, procedures, and techniques used in the development of Individual Support Plans (ISP)
- Knowledge of principles and practices of case management
- Skilled in oral and written communication
- Skilled in developing, evaluating, and maintaining member service plans
- Skilled in interviewing, counseling, and interacting with members and family members
- Ability to work independently and within diverse work teams
- Ability to document case files and information within required/mandated timelines
- Ability to maintain confidentiality
- Candidate must be an Arizona licensed Registered Nurse in good standing OR possess a Bachelor Degree in Social Work (SW), OR possess a Bachelor’s degree in Psychology, Special Education or Counseling AND have at least one (1) year experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI) OR Candidate must have a minimum of two (2) years’ experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI). For Case Managers who will serve persons who are elderly and/or persons with physical or developmental disabilities and determined to have an SMI the requirement is as follows: One (1) year of case management experience serving elderly and/or persons with physical or developmental disabilities, AND Two (2) years of case management experience serving members determined to have an SMI
- A Bachelor’s degree in Human Service, Behavioral Science or related field AND have at least one (1) year experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI) OR Associate degree in Human Service, Behavioral Science or related field AND have at least one (1) year experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI) OR have at least three (3) years’ experience working with or for individuals with intellectual or developmental disabilities.
- This position requires possession of and ability to retain a current, valid state-issued driver’s license appropriate to the assignment. Employees who drive on state business are subject to driver’s license record checks, must maintain acceptable driving records and must complete any required driver training (see Arizona Administrative Code R2-10-207.12.). Employees may be required to use their own transportation as well as maintaining valid motor vehicle insurance and current Arizona vehicle registration; however, mileage will be reimbursed.
- Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults.
- Candidates for this position shall be subject to a search the Child Protective Services Central Registry pursuant to A.R.S. 8-804.
- Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees
- Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency’s ability to reasonably accommodate any restrictions

Critical Services Premium Pay Stipend
This position may be eligible to receive a temporary stipend of 10% in addition to base pay beginning in December 2021. If eligible, the temporary stipend amount will be calculated using the employee's base rate of pay and will be reflected in each paycheck. It is anticipated that this temporary stipend will be available through June 24, 2022.
