This position will require travel to residential (group homes) and non- residential settings (ADTs) in Miami Dade and/or Monroe counties.
The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children ages 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities. SPECIFIC DUTIES AND RESPONSIBILITIES
Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service. This includes, but is not limited to, always acting in a responsive, professional, courteous manner with the employees, customers, and public we serve at all times.
Duties and responsibilities include, but are not limited to, the following:
The primary function of this position is ensuring APD residential (group home and assisted living facility) and non-residential (adult day training and prevocational) setting compliance with the Centers for Medicare & Medicaid Services (CMS) requirements, set forth in 42 CFR 441.301. The duties to be performed include:
Conduct onsite monitoring through established survey protocols of APD residential and non-residential settings located within the region to determine compliance with 42 CFR 441.301(c)(4).
Onsite monitoring includes conducting interviews of individuals served within residential settings and non-residential settings to verify person centered plans are in place that ensure rights, dignity, choice, and community integration.
Conduct validation surveys of residential and non-residential settings every three years to ensure continued compliance with CMS requirements.
Complete written survey reports summarizing findings related to the providers’ compliance with CMS requirements, including identification of deficiencies to be remediated.
Coordinate with providers on completion of plans of remediation/correction and provide technical assistance for providers to come into compliance with CMS requirements.
Conduct follow-up surveys to ensure providers complete their plans of remediation within established time frames.
Participate in meetings to discuss concerns and provider actions that may be necessary, including terminations.
Conduct heightened scrutiny reviews of settings that appear to meet the definition of presumptively institutional and report findings to the APD leadership team and participate, as necessary, in the review of public comments on settings identified as presumptively institutional, providing recommendations on identified issues as requested.
Provide APD leadership team with status reports of completed assessments.
Participate in all training and technical assistance conference calls.
Perform other duties as assigned.

BACKGROUND SCREENING REQUIREMENT
It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.
No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.
Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement, and Federal criminal records checks through the Federal Bureau of Investigation and may include local criminal records checks through local law enforcement agencies.
Pursuant to S. 110.1127(1), Florida Statutes, this position requires a background investigation, including fingerprinting.
