Your Specific Responsibilities:
This is a high level position that is responsible for serving as the contract administrator, grant writer, and coordinator of the Strategic Plan, Quality Improvement Plan, Workforce Development Plan and other health planing activities for the Flagler County Health Department. The incumbant must demonstrate good communication and organizational skills with planning abilities and strong writing skills. Position is under the direct supervision of the CHD Administrator.

Facilitate and manage the health planning process that includes facilitation, development, implementation and evaluation of the triannual Community Health Assessment CHA) and Community Health Improvement Plan (CHIP). This task is coordinated with several community partners including Flagler Cares, Inc. and Advent Health Palm Coast hospital. Also, this position involves the coordination, development and evaluation of the Strategic Plan, Quality Improvement Plan and Workforce Development Plan that are required to maintain accreditation by the Public Health Accreditation Board:
Incumbent to help with medical supply testing annually, bi-annually, and quarterly to include updating records/logs using excel. Reporting will include communication with medical equipment supply companies if necessary, depending on testing results.
Incumbent is required to complete time sheet, work activity reports (EARs) if applicable, and mandatory trainings. Trainings may be deemed mandatory by Agency, Statute, Grant, Program, or County Administrator. Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. Must be able to lift 50 pounds or more.

Qualifications:
Minimum -
Are you willing to work before, during, and/or beyond normal work hours or days in the event of an emergency? This may include working in a special needs or Red Cross shelter, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural?
Do you have experience in Public Speaking?
Do you have experience in leading groups toward consensus?
Demonstrated work experience with writing a QI plan, Strategic Plan or Business Plan or comparable plan?
Do you have experience in facilitation/preparation of either a QI Plan, Strategic Plan, Business Plan or comparable plan?
Preferred:
Do you have a Associates degree from a reputable college or university?
