Your Specific Responsibilities:

This is a high level position that is responsible for serving as the contract administrator, grant writer, and coordinator of the Strategic Plan, Quality Improvement Plan, Workforce Development Plan and other health planing activities for the Flagler County Health Department. The incumbant must demonstrate good communication and organizational skills with planning abilities and strong writing skills.  Position is under the direct supervision of the CHD Administrator.

 

Contract Administrator:

  1. The responsilities include negotiating, drafting, writing, executing, monitoring, processing and tracking contract payments, maintaining contract file, and closing out contract file .
  2. Enters all two-party agreements into the Department of Health contract repository and the Florida Accountability Contract Tracking System (FACTS).
  3. The duties include following the DOH Contractual Policy and ensuring contracts are in compliance with applicable state and Federal laws, rules, and regulations governing contracts for services.
  4. Coordinates contract procurement. Ensures that programmatic and administrative monitoring is done as required. Conducts site visits if needed with the contract mananger as required. Works with contract managers to approves invoices to turn in to fiscal office for processing and maintains records of payments made.
  5. Serve as Contract Liaison to the Office of Contracts and ensures consortium two-party agreements are submitted according to policy I0P58-01-21. Follows all established procedures for the review, approval, and execution of two-party agreements.

 

Grant Writing:

    1. Study and understand the history, structure, objectives, programs and financial needs of the organization.
    2. Research grant opportunities from government and non-government agencies.
    3. Draft grant proposals and supporting documents based on the funding requirements of the organization.
    4. Submit proposals to grant coordinators for approval.
    5. Respond to internal and external queries on drafted and submitted proposals.
    6. Maintain positive relationships with fund providers and other stakeholders.
    7. Maintain records and submit reports related to grant opportunities.

 

Facilitate and manage the health planning process that includes facilitation, development,  implementation and evaluation of the triannual Community Health Assessment CHA) and Community Health Improvement Plan (CHIP). This task is coordinated with several community partners including Flagler Cares, Inc. and Advent Health Palm Coast hospital. Also, this position involves the coordination, development and evaluation of the Strategic Plan, Quality Improvement Plan and Workforce Development Plan that are required to maintain accreditation by the  Public Health Accreditation Board:

 

  1. Quality Improvement:
  1. Works with departmental leaders for the development of a quality improvement teams, serving as an active member of each team.  Assists with executive summary composing and submission. Assists the teams in quality improvement projects.
  2. Develops QA/QI Plan and ensures consistency with Accreditation and Strategic Plan Initiatives.
  3. Attends scheduled meetings of the Performance Management Council.
  4. Proficiency in understanding and obtaining data from Florida Charts and other community profile databases.
  5. Reviews performance indicators, evaluates trends and recommends specific measures for improvement.
  6. Utilizes Process Management for optimal customer service and resource utilization.

  1. This is a 5-year plan that includes quarterly objective updates, annual revisions and progress report.

  1. This is a 5-year plan that includes quarterly objective updates, annual revisions and progress report.
  2. Track and maintain the Individual Development Plans for each employee in the CHD.

      

Incumbent to help with medical supply testing annually, bi-annually, and quarterly to include updating records/logs using excel. Reporting will include communication with medical equipment supply companies if necessary, depending on testing results.  

Incumbent is required to complete time sheet, work activity reports (EARs) if applicable, and mandatory trainings. Trainings may be deemed mandatory by Agency, Statute, Grant, Program, or County Administrator. Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. Must be able to lift 50 pounds or more.

  • Must be proficient in Microsoft Office programs (Word, Excel, Outlook, and PowerPoint).
  • Ability to meet deadlines without sacrificing accuracy or customer service.

Qualifications:

Minimum -

Are you willing to work before, during, and/or beyond normal work hours or days in the event of an emergency? This may include working in a special needs or Red Cross shelter, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural?                                                                                                                                          

Do you have experience in Public Speaking? 

Do you have experience in leading groups toward consensus? 

Demonstrated work experience with writing a QI plan, Strategic Plan or Business Plan or comparable plan?        

Do you have experience in facilitation/preparation of either a QI Plan, Strategic Plan, Business Plan or comparable plan? 

 

Preferred:

Do you have a Associates degree from a reputable college or university?
