Healthcare Social Worker
Seeking a Healthcare Social Worker in Kingsport, TN.

PRIMARY RESPONSIBILITIES:
- Collaborate with the case management team in the delivery of care for patients with chronic, acute, or terminal illnesses.
- Perform visits/consults to assess psychosocial needs, patient safety, housing, food, medication, etc.
- Identify environmental impediments to the patient’s progress.
- Match/refer patients, family members, and other caregivers to community resources and services.
- Assist with planning discharge from care facility to home or other care facility.
- Modify the treatment plan with changes to patient status.
- Empathizes and advocates for patients, family members, or other caregivers.
- Organize support groups or council family members.
- Investigate abuse or neglect cases.
- Maintain strict confidentiality.
- Follow organizational policies and procedures.
REQUIREMENTS:
Three or more years of experience in healthcare social work.
EXPERIENCE:
A minimum of three years of related experience in utilizing, maintaining, and the administration of an EMR within a physician practice setting. Knowledge of typical physician office workflows and processes in both clinical and clerical areas is required.
CERTIFICATES, LICENSES, REGISTRATIONS:
Valid driver's license with appropriate insurance coverage and acceptable driving record.
