Travel is required within the St. Louis County service area

Role Description
We are looking for someone who can be committed to our core competencies. As an Associate/Social Service Specialist at the Division of Developmental Disabilities (DDD) we need someone who has skills and knowledge to support persons with Developmental Disabilities and their families that are consistent with State and national policies and practices.
What you’ll do
- Be a knowledgeable, adaptable and ethical professional.
- Develop and maintain a relationship with the individual and their team to facilitate effective communication and collaboration to promote well-being.
- Enhance the individual’s capacity for self-direction through ensuring awareness of rights and responsibilities, and facilitating access to resources.
- Engage the team in a person-centered planning process that results in an integrated and comprehensive plan that is reflective of and responsive to the strengths, interests, needs, and desired outcomes of the individual in all areas of their life.
- Assist the support team to cultivate an array of resources that meet the needs of the person, including paid and non-paid supports.
- Facilitate the exploration and acquisition of paid supports from a variety of funding sources. Conduct in-person visits to monitor for quality services to ensure the services maximize the use of support dollars to meet identified goals and minimize risks.

Minimum Qualifications
Bachelor’s degree; or a Bachelor’s Degree and 1-2 years of relevant experience; or a Registered Nurse.
