This position is located at the Department of Mental Health, Patric B. Harris Psychiatric Hospital, 130 Highway 252, Anderson, South Carolina 29621.
As the Nurse Practitioner, working under the supervision of the physician preceptor, provides Psychiatric/ Medical management to patients of Harris Hospital, you will provide primary care to patients with complex, acute, critical and chronic health conditions, including the delivery of care to patients experiencing episodic illness, exacerbation of chronic illness, or terminal illness and emergency medical care and/or crisis behavior management. This role is an integral member of the interdisciplinary health team. Acts as a resource person to nursing and allied health personnel.
Responsibilities include but are not limited to:- Assumes responsibility for the care of assigned patients in collaboration with the physician preceptor and other members of the interdisciplinary health team. Performs assessments, orders, interprets, performs diagnostic tests, provides plan of care and treatments to patients with acute and chronic condition. Conducts daily rounds and participates in treatment team meetings. Coordinates continuity of care to clients in collaboration with the physician preceptor.
- Provides leadership in the patient care management through evaluation, modification, and documentation including assessment of the patient's response to therapy, changes in condition, and therapeutic interventions to optimize patient outcomes. Analyzes the indications, contraindications, risk of complications, and cost-benefits of therapeutic interventions. Performs reassessments when patient's condition warrants and performs annual reassessments on assigned patients per policy. Refers HPH patients for emergency or specialty consultation as indicated.
- Applies principles of crisis management in assisting the patient and family experiencing complex acute, critical, and chronic illness. Creates a climate of mutual trust and establishes partnerships with patients. Functions as patient advocate.
- Participates in peer review activities and performance improvement activities, monitoring and ensuring the quality of health care practice. Attends meetings as assigned.
- Promotes valuing of lifelong learning and evidence-based practice while continually acquiring knowledge. Participates in the continuing education of the nursing staff. Maintains Licensure/Certification Educational requirements and mandatory training requirements.
- Demonstrates knowledge of relevant legal regulations for nurse practitioner practice. Adheres to HIPPA and Corporate Compliance requirements. Maintains competency in MAPS, CPR, and SCEIS Leaming Modules.
State Requirement: Graduation from an accredited school of nursing at the master's level or doctoral level acceptable to the Board to include clinical experience.Agency Requirement: Requires a nursing license and official recognition by the South Carolina State Board of Nursing as a Nurse Practitioner practicing in the extended role with authorized prescriptive authority in South Carolina.
