What are the responsibilities and job description for the Social Worker - Hospice position at Stein Hospice?
Position Summary:
Functions as an integral member of the interdisciplinary team by providing direct social work services to patients/families and social work consultation to other team members. Services include: assessment, counseling, emotional support, coordination of assistance for financial/legal affairs, and plans for care giving.
Qualifications:
- Bachelor’s degree in social work from a school of social work accredited by the Council of Social Work Education with at least one (1) year’s work experience in social work in a health related facility preferred.
- Licensed by the State of Ohio Counselor and Social Work Board.
- Demonstrates the ability to be supportive and sensitive to patient/family needs.
- Skills must include written and oral communications, organization and documentation
- Ability to practice in accordance with Stein Hospice’s clinical standards of practice, policies and procedures ·
- Knowledge of community and social services resources.
- Must be flexible and have problem solving skills, prioritize and demonstrate healthy coping strategies
- Assists team, attending physicians, staff, patients and families in the clarification of treatment goals and the plan of care.
