Responsibilities
Under the direction of the home care manager, the RN case manager performs primary nursing care functions in conformance with professional standards of home care nursing practice under the plan of care approved by the primary physician. The home care nurse coordinates all services provided for each assigned patient and family.

Requirements
- RN licensure in the State of Maryland
- Prior experience in hospice or home health strongly preferred
- Valid driver’s license and reliable transportation
- Commitment to Stella Maris Hospice’s philosophy of care, values, mission statement and organizational culture
- Willingness to serve on-call
**This position requires daily travel in assigned region**
