Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will provide support and guidance to individuals and families in need. Your role will involve assessing clients' needs, developing care plans, coordinating services, advocating for well-being, and coaching clients for their well-being.
The ideal candidate will have experience in motivational interviewing, counseling techniques, time-management related to workforce development, housing counseling, child welfare laws, and crisis intervention.
Duties:
- Conduct outreach, intake, and coordinate comprehensive assessments to service linkage on identifying clients' needs and develop individualized care plans.
- Conduct face to face interviews. Schedule client review and update information as needed.
- Coordinate, follow-up, and monitor services provided to clients, ensuring they receive appropriate support and resources for outcomes.
- Collaborate with other professionals, such as social workers and healthcare providers, to ensure a holistic approach to client care.
- Provide counseling and guidance to clients on various topics, such as addiction recovery, job readiness, housing assistance (Section 8), hud rapid re-housing assistance, and accessing healthcare (Medicare) needs and services.
- Assist clients in navigating the social service system and connecting them with community resources
- Conduct regular follow-ups with clients to monitor progress and make necessary adjustments to care plans
- Provide crisis intervention as needed to ensure the safety and well-being of clients
- Supervise and mentor case management staff, providing guidance and support in their professional development
Qualifications:
- Bachelor's degree to Entry level in social work or a related field.
- Proven experience in case management or a related field
- Knowledge of motivational interviewing techniques and behavioral health principles
- Familiarity with workforce development programs and resources
- Familiar with community services and housing resources
- Understanding of child welfare policies and procedures
- Strong organizational skills with the ability to prioritize tasks effectively
- Excellent communication skills, both written and verbal
- Ability to work collaboratively with a diverse range of individuals and organizations
