Who We Are
Steps to Recovery is a premier substance use disorder and co-occurring mental health disorder outpatient center. For both substance use disorder and mental health clients, we will offer separate programs for each population. Our programs will include partial hospitalization treatment, intensive outpatient treatment (day and night programs), and outpatient care, all with the support of a psychiatric provider, licensed clinicians, and experienced professionals. Medication-assisted treatment (MAT) options and psychiatric medication management will be offered at all care levels. Our programs are trauma-informed, individualized, and accredited by the Human Rights Campaign as a leader in providing LGBTQ services.

The care manager will work Monday/Wednesday/Friday 9a-5pm and Tuesday/Thursday 1p-9p
Position Summary
Position Responsibilities
1. Working 40 plus hours a week
2. Acting communication link between designated facilities, client families, referral sources and STR
3. Conducting and completing intakes for all new clients admitting to STR
4. Receiving and managing phone call inquiries from prospective clients, families and professionals
5. Providing regular family updates via phone, email
6. Scheduling appointments and transportation for clients as necessary
7. Reviewing, filing, an updating client-related forms on EMR;
a. Consents
b. Intake forms
c. Family Contact Forms
d. Referral Information
e. Follow up contact with clients and family
8. Manage issues related to FMLA, EAP, Disability, Legal, Financial, Personal Therapy, etc.
9. Complete prior authorizations as needed for client medication fills
10. Communicate with specialty pharmacies for injection medication ordering, refill and delivery
11. Regular quality assurance checks with clients’ family and referral sources
12. Meet with all discharge clients to complete discharge summaries and coordinate Aftercare Plans
13. Manage E-basis assessments as scheduled
14. Collect client satisfaction surveys
15. Complete follow up calls with discharged client to assess their aftercare plan participation and relapse risk level
16. Communicate and assist in re-admission for discharged clients if needed
17. Facilitate clinical groups as necessary and assigned
18. APA interventions as needed
19. Perform other duties as assigned
