Compensation starting: $68,000 - $94,000/year based on experience and education.

Responsible for managing the care clients receive in their homes.

The Certified Care Manager performs a variety of duties that relate to care management including assessments, care planning, coordination of services, communication, documentation, home and physician visits, travel, preparation of reports, errands, internet research and a consultative approach to determine each client’s needs to provide solutions and create a tailored service plan. This position enables clients to live in their homes longer and ease the minds of family members knowing their loved one is cared for.

Candidates must possess a nursing license for RN and be eligible to practice as an RN in the state of SC. Excellent computer skills and a current driver’s license is required. Five years healthcare experience and/or five years demonstrated experience working with seniors, people with dementia or people with complex medical needs preferred. Candidates must be professional and have a positive approach, commitment to customer service, initiative-taking, and work with own initiative, strong in building relationships, team player and able to communicate at all levels.
