What are the responsibilities and job description for the VNA RN Case Manager - $$ Sign On Bonus position at Stone Senior Living?

Candidates looking to join our team need to be licensed RN with recent clinical experience.
Responsibilities:
- Ability to travel throughout the specified area
- Provide skilled nursing care – assessing, teaching, counseling and appropriate preventative and rehabilitative nursing measures
- Conduct an initial and on-going assessment of the patient’s physical, psychological and social needs
- Complete and submit all necessary documentation
- Communicate pertinent information regarding patients’ care and need
- Provide individualized patient care utilizing professional knowledge
Requirements:
- Current Massachusetts RN license
- Valid Driver’s License
- Current CPR Certification
- Recent clinical experience
- I.V. experience preferred but not required
