What You Will Do as a Social Worker

As a Social Worker, you will collaborate with the centers' interdisciplinary team to ensure admissions, discharges, and social needs are planned, assessed, and met. Some of your primary responsibilities include:
- Conducting assessments and gathering information that promotes development and implementation of a social program that meets needs of short-term and long-term patients.
- Completing assessments, including MDS, Social Service Assessment and other documentation required by regulations, policies and procedures, and/or standards of practice.
- Working with the interdisciplinary team to identify opportunities to improve care and services, including participating in care plans, QAPI, and other committees as assigned.
- Developing processes and procedures to ensure patients are informed of their rights, services, and community resources.
- Providing training and materials to promote continuity of processes involving social support, abuse/neglect reporting and investigations, maintaining patient rights, and promoting dignity.
- Providing input into and assisting in developing policies and procedures that reflect the standards and mission of the center and Stonerise.
- Addressing concerns, complaints, and grievances brought forth by patients, visitors, and other parties. Communicating information to the Administrator and appropriate managers, conducting investigations, documenting actions and outcomes.

Schedule
Full-Time, Non-Exempt
Education, Licensure & Certification Requirements - Baccalaureate degree in Social Services or equivalent degree in human services field with a WV Social Worker license.
- Minimum of one (1) year of experience in Social Services position in a healthcare setting.
