POSITION SUMMARY
Upon completion of orientation and under general supervision, Assessment & Referral Specialist performs assessments at SBH as well as outside the facilities, depending on the needs of the patient or hospital, screens admissions for clinical and medical appropriateness and communicates this information to a physician for determination of admissions and level of care direction. Works closely with business office, nursing and clinical staff as well as external parties to ensure the timely admission of patients.
Education / Licensure:
Position requires successful completion of a master’s degree from an accredited college or university in social work, psychology, mental health or a related field or RN. LPCC, LMFT, or LMSW preferred. A minimum of two years in a psychiatric or mental health setting desirable. Must have a valid State driver’s license.
Candidate will possess a valid current registered nursing license from the State Board of Nursing or from a nurse compact state, where applicable.
ESSENTIAL JOB FUNCTION AND RESPONSIBILITIES
Customer Service
1. In-person and telephone communication is consistently attentive, responsive and reflects “helpfulness”.
2. Professional awareness and boundaries are observed. Unnecessary self-disclosure is minimized.
3. Telephones are answered by the third ring whenever possible and a professional greeting. Dedicated extensions/mobile phones with voice mail have a personalized greeting, and messages retrieved/responded to in a timely manner.
4. Critical customer encounters are met with advanced customer service skills and resolved.
Assessment Services
1. Coordinates activities related to admission of patients and appropriate follow-up activity.
a. Coordinates activities related to admission of patients and appropriate treatment planning.
b. Coordinates treatment plan based on clinical assessment using evidenced based tools and treatment modality.
2. Conduct initial evaluation to include interviewing patient and/or patient’s representative to obtain necessary personal, medical and psychiatric history and financial data to determine eligibility for admission, appropriate level of care and document recommendations based on DSM 5.
3. Confers with physicians, nursing, housekeeping, transport and other staff members to coordinate and schedule admission of patient. Assigns accommodations based on physician’s admittance orders, nature of illness, availability of accommodations and other information.
4. Prepares record of admission, transfer and other required data.
5. Notifies departments of patient’s admission. Maintain record of admissions and discharges and complies occupancy-census data.
