Summary:
Ensures timely, system-wide access to comprehensive, quality healthcare for patients with injuries sustained from trauma. Facilitates the care of the complex Trauma patient through timely communication between healthcare team members, collaborating with physicians, nurses, social workers and other team members. Serves as the primary patient/family educator and advocate ensuring the patient receives optimal care and services throughout the transition from inpatient to outpatient, within the health care system. Provides access to information and resources to empower patients to self-manage care. The goal of this role is to alleviate barriers to care, decrease hospital length of stay, improve adherence to treatment regimen, reduce readmissions, improve patient access to care for the promotion of quality, cost effective outcomes.
Formal Education Required:
a. Bachelor of Science in Nursing
b. Registered Nurse with current license to practice in the State of Ohio
Experience and Training Required:
a. Two (2) years of experience in nursing care
b. Experience should include adult care requiring management of multiple co-morbid illnesses, care of trauma patient population preferred
c. TNCC or TCAR certification preferred

e. Knowledge of electronic medical record.
f. Ability to analyze, use clinical research outcomes, and aggregate data effectively
g. Ability to be open minded, objective and self-directed in executing responsibilities within a team structure. Proactive approach to problem solving.
Level of Physical Demands:
a. Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently.
b. Ability to walk long distances within the hospital campus.
c. Ability to sit and/or stand for long periods of time.
d. Manual dexterity to operate computer, phone, and standard office machines.
$30.60/hr - $45.90/hr
The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay. We believe in the importance of pay equity and consider internal equity of our current team members when determining offers.
