The Summit Community Care Clinic (SCCC) Health Coach (HC) will promote, maintain, and improve individual and community health by supporting patients in adopting healthy behaviors. The Health Coach shall utilize clinic data to drive programmatic decision-making. This person will be responsible for tracking quality assurance measures, reducing barriers to accessing care, and identifying the needs of our patients and the clinic.
The Health Coach will manage the development and implementation of health education materials. They will facilitate related support groups, implement health education courses, and conduct outreach programs. They may facilitate projects or activities related to attaining or maintaining accreditation, conduct or participate in research projects related to health issues, and provide skill training related to health issues.
Essential Functions:
- Review charts of medical providers to determine which programs and resources patients qualify for. Communicate with providers regarding referrals and services needed.

Determine “Action Plans” & follow-ups based on patient-driven health goals. For example, Wise Women focus areas (cardiovascular, cervical, and breast health), Tobacco and Nicotine Cessation areas, or weight management.

Manage the Health Coaching internal referrals, responsible for patient outreach, scheduling, follow-up, and “closing the loop.”

Thorough/accurate documentation in the patient’s electronic health record. Documentation must contain quality information reflecting their patient encounter.

Use resources available to anticipate, address, and overcome barriers to care and guide patients through the health care system. (Barriers to quality care may include financial/economic, linguistic, and cultural communication, the system's complexity, transportation fear, and bias based on culture/race/age.)

May assist with identifying local resources (including elder or childcare), communicating with health care professionals, and making appropriate referrals.
- HCs may be responsible for maintaining and managing patient registries such as prenatal, diabetes, hypertension, or Wise Women.

Discuss lab results after review by the medical team with patients to collaborate on goal setting to improve health outcomes.

It may be necessary to use case management and patient navigation to ensure the patient successfully attends their visits. (Case management is used when necessary to coordinate patient care services to improve the quality of the total patient experience.)

Collaborates with treatment team members, such as the referral coordinator, interpreter, or nursing, to facilitate the best care for patients.

Utilizes evidence-based practices (EBP) and materials when creating resources for patients.
- Control and maintain approved materials and resources in a predetermined location.
- Provide presentations on general health, disease-related topics, and up-to-date information about health and wellness, within the scope of practice and relevance to the Health Coaching program.

Provide and implement culturally competent interventions to improve the quality of health care for patients when needed, which may include, but are not limited to interpretation services, developing health information for patients that is written at the appropriate literacy level and is targeted to the language and cultural norms of specific populations.
- Define and maintain clear boundaries, roles, and functions with patients and providers.
- HCs are not to practice outside their scope of practice. Scope of Practice does not include physical assessments, diagnosing, prescribing, medication adjustments, changing dressings, providing direct financial assistance, picking up patients for appointments, offering opinions about any aspect of health care delivered, or providing recommendations or opinions about physicians and/or health care organizations.
