JOB DESCRIPTION

• Conduct assessments to determine client treatment needs, in person, telehealth, phone in a variety of different locations.
• Provide individual and group counseling, using variety of therapeutic techniques, to facilitate and maintain sobriety and wellness, to assist in growth and progress toward completion of treatment plan goals and objectives
• Develop materials for and facilitate client group meetings, relapse prevention, trauma, parenting, spirituality, etc., to assist in sobriety and wellness
• Assess client progress to evaluate success of treatment
• Maintains records and documents on clients and treatment progress as required by Federal, state, insurance/payment provider and Summit Counseling Services standards, regulations and policies
• Attend and participate in staff meetings to consult with other professional staff to ensure appropriate treatment, programming and client progress
• Refer clients to and consult with other support services as needed to ensure continued and appropriate treatment
• Conduct administrative duties to include progress notes and documentation, creation and review of treatment plans, completed in a timely manner according to program-specific policy and procedure, to adhere to applicable state, federal and agency regulations
• Provide and maintain reliable and prompt attendance, with the ability to work the schedule dictated by business necessity, to include evenings, on-call rotation and weekends as needed
