Responsibilities
Program/Patient Oversight
- Conduct program education and outreach to stakeholders, providers, and the public
- Develop/maintain materials (print or otherwise) that provide guidance for program implementation
- Facilitate monthly meetings with Assisted Outpatient Treatment (AOT) team for program planning and evaluation purposes
- Serve as a liaison to local mental health organization, advocacy groups, and the court
- Review referrals and treatment plans for AOT participants to ensure meeting the requirements
- Work closely with research evaluators to improve data collection and ensure data is informing improvements in program implementation
- Develop and submit mandatory reports on AOT activities annually
- Work under the guidance and direction of SAMHSA's grant program officers and within the scope of Calhoun County’s approved grant for AOT
- Work on program sustainability by identifying and assisting with proposals/grants for additional funding sources such as legislative appropriations and other government funding
- Maintain organized electronic and paper records of AOT activities, events, and data sometimes including sensitive or private health information.
- Participate in training and professional development opportunities such as AOT grantee meetings, webinars, and trainings on civil commitment laws.
- Participates in Crisis Stabilization Team (CST) home/community visits to assist customer re-engage in services using Motivational Interviewer (MI) techniques
- Demonstrates good organizational, verbal/written communication, computer, analytical skills.
- Shall be self-directed and resourceful. Participates in corporate quality improvement and problem solving activities. Maintains confidentiality and privacy within MDHHS, state and federal guidelines. Utilizes knowledge of CARF, CMS, MDHHS and Managed Care standards.
Contract and Grant Coordination
- Coordinate the development of Request for Applications (RFA), Request for Proposals
- (RFP), contracts, or other instruments used by the program in obtaining services.
- Develop schedules, performance measures, and evaluation methods for contracts.
- Track all contracts and amendments through to execution.
- Approve reports and invoices in accordance with contract requirements and county policy.
- Responsible for guiding the department and contractors to meet grants requirements.
- Submit reports, applications, and data as requested by the funding agency.
- Oversee data collection and reporting in accordance with funding agency.
Qualifications
- Licensure in the State of Michigan. Minimum licensing requirements LLBSW/LBSW. Other behavioral health licensures also accepted (LLPC/LPC, TLLP/LLP, LLMSW/LMSW).
- Two years of experience in behavioral health care setting as a practicing licensed professional preferred. Experience working with individuals who have a severe and persistent mental illness preferred. Prefer QBHP requirements met: One year of specialized training or experience in treating or working with a person who has a mental illness (QBHP).
