Responsibilities:
- Administer medications and treatments to patients according to physician orders
- Monitor and assess patients' conditions, communicate changes to the appropriate healthcare professionals, and provide appropriate interventions
- Collaborate with other members of the healthcare team to develop and implement patient care plans
- Provide patient education and counseling on health-related issues
- Assist in the coordination of patient care, including scheduling appointments, arranging for referrals, and ordering necessary supplies
- Maintain accurate and up-to-date patient records and documentation
- Follow all nursing policies, procedures, and protocols to ensure patient safety and quality care
- Participate in performance improvement initiatives and maintain professional development and competency
Qualifications:
- Current and valid LVN license in the state of [State Name]
