Our Inwood Health Center was founded in response to NYC’s homeless and HIV crisis in the Bronx. This Community Health Center is dedicated to the critical and compelling work of caring for NYC’s homeless community. The center provided life changing access to primary care for 1,000 patients in need of chronic care management, HIV, urgent care, substance use services, mental health care, Hep C and coordination & advocacy. The multi-disciplinary team is exceptional at engaging patients who have been without care for far too long.
We are looking for a mission driven, creative and compassionate primary care provider to join the movement and make a difference!
The health center is located in High Bridge / Tremont area of the Bronx and works with shelters throughout the city as homeless individuals move through the NYC Homeless Service program. It was the 1st CHC location of HELP PSII Brightpoint now Sun River as the mission of the HIV care moved from nursing home care to outpatient care, early detection, and prevention. Care of patients with HIV and helping to achieve viral load suppressor for those with complex needs remain an ongoing focus.
Central Bronx location – easy commute from Manhattan, Westchester and New Jersey!
Full and part time positions. Job share and location share options available.

Pay : $145,197.00 - $152,236.50 per year
