Sun River Health is a Federally Qualified, Non-For-Profit Health Center serving communities in Suffolk County. We currently have the need for a full-time PrEP Navigator to join our team in Peekskill, Beacon, Poughkeepsie, Nyack and Spring Valley.
Identify and educate high risk patients about PrEP implement strategies to encourage linkage and retention in care.
Essential Job Duties and Responsibilities:
- Identify and educate high risk patients about PrEP implement strategies to encourage linkage and retention in care.
- Utilize lived experience as a teaching tool to educate patients on PrEP regarding medication adherence, sexual health, medical follow up, and reducing barriers to care.
- Monitor patients receiving PrEP using excel spreadsheets, dashboards and other software to evaluate services.
- Conduct community outreach engaging and screening at risk patients in PrEP treatment.
- Assist PrEP Specialist in conducting patient education sessions on risk education, safer sex, STI prevention and PrEP.
- Conduct community presentations.
- Attend collaboratives and community events professionally representing HRHCare.
- Document patient services in electronic health record.
Minimum Education Requirement:
Associates Degree
Minimum Related Work Experience:
2 - 4 years
Minimum Specialized Skills & Knowledge Requirement:
Ability to communicate in English (oral and written)
Hourly Rate: $16.75-$18.25/ hour
Salary : $17 - $18
