Position Summary:
The successful LPN candidate will thrive in a dynamic, fast-paced rehabilitation and nursing center. Adaptability is key, as the role requires the ability to quickly familiarize oneself with various software applications.
Responsibilities:
- Conduct comprehensive evaluations of residents' rehabilitation needs.
- Collaborate with medical professionals to plan and execute resident care in alignment with physician orders.
- Implement and oversee resident care plans, regularly evaluating and updating as necessary (PRN).
- Administer medications to residents according to prescribed schedules.
- Monitor vital signs, including blood glucose levels, temperature, and blood pressure.
- Assist residents with Daily Living Activities, including bathing, dressing, grooming, eating, transfer, ambulation, toileting, and other personalized care needs.
- Supervise and ensure Certified Nursing Assistants (CNAs) adhere to physician orders.
Qualifications:
- Must be currently licensed in the State of New York as a Licensed Practical Nurse.
- Must be CPR Certified.
