Responsibilities of the Case Manager
- Provide supervision for assigned residents
- Identify special resident problems and report them immediately to charge nurse or DNS as appropriate.
- Provide direct resident care as assigned
- Assist with the implementation of treatments and updating the care plan
- Identify safety hazards and emergency situations and initiates corrective action immediately
- Manage the appropriate use of all equipment, supplies and resources within the facility.
- Monitor residents’ medication compliance as directed by Nurse Manager.
- Inform Nurse Manager of unusual events or resident behavior in a timely fashion.
- Comply with all state WAC requirements and facility policies and procedures.
- Monitors cleaning schedules, POC and PCC.
- Helps with resident issues and med room compliance.
- Performs pharmacy orders and records/receipts
- Checks EMAR for holes in the MAR monthly.
- Sends record requests to different facilities.
- Covers NAC shifts as needed.
- Serves as on call for staff questions during week/weekend.
Education/Experience:
- High School Diploma or General Education Degree
- Successful completion of a Washington state-approved Nursing Assistant Certification course or other education which meets state certification requirements.
- 2 years in staffing/management experience preferred but not required.
Certificates/Licenses:
- Current Nursing Assistant Certification in Washington or be eligible to obtain certification within four months of employment.
- Medication delegation and diabetes certification
- Current CPR certification and First Aid
- Valid Washington State Driver’s License with a satisfactory driving record.
- Eligibility for Washington State registration.
- Food Handler’s Permit required (unless working night shift)
