Job Summary:
The Patient Care Specialist (Licensed Practical Nurse) will work under the direction of a physician providing care and symptom management to cancer patients. Must encompass the roles of direct caregiver, monitoring side effects of and tolerance to toxicities of radiation with any prescribed combined chemotherapy. This includes the clinical nursing component of establishing a plan of care, directing patient care, patient education, serving as a patient advocate, and documentation of nursing interventions and outcomes.
Duties to include:
- Prepare ahead for all new patient, on-treatment, and follow-up appointments to include review of chart for updates; request any needed images and reports; anticipate any test that each respective physician may require before seeing patient; Ensure all pertinent test results and reports are obtained prior to scheduled appointment.
- Conduct weekly and PRN nursing documentation in medical record to include nursing assessment of patient tolerance to radiation and/or chemotherapy toxicities, instruction/teaching, and nursing and physician interventions; document patient/family responses, telephone messages, medication updates in medical record; Communicate and document any patient problems encountered daily.
- Provide support to front desk staff with covering phones and other tasks that may be required.
- Demonstrate a willingness to be a team player; Establish and maintain positive working relationships with patient, family, physicians and other departmental staff.
Minimum Qualifications:
Licensed Practical Nursing Certificate. NYS licensed/registered LPN. Current BLS-CPR certification required at the time of appointment.
Preferred Qualifications:
Oncology experience is preferred.
Work Days:
Monday - Friday, 8:00 am - 4:30 pm

This position is primarily based in Syracuse with 1-2 days coverage at our Oswego office. Mileage reimbursement provided for travel to our Oswego office.
