REMOTE IN TEXAS. Applicants must reside within 60 miles of a Superior Health Plan office in Texas.Asa Behavioral Care Coordinator, you will:
Conduct screening and risk assessment interventions per program guidelines. Provide outreach and preventative care to members seeking services in the least restrictive environment.• Identify special needs members through the completion of health screens and other resources.
• Educate providers, members and their families on behavioral health issues including symptoms, relapse prevention, stress reduction, lab work, and healthy lifestyle choices.
• Coordinate behavioral health treatment with PCP’s and behavioral health providers.
• Participate in the development and modification of care plans in conjunction with member, member’s family and managing physician to include, monitoring for treatment compliance and assisting with problem solving in breaking down barriers to treatment to include referrals to community resources.
• Coordinate urgent care evaluations when necessary.
• Assist members with locating providers and scheduling appointments as necessary.
• Assist with development of plan specific literature and education materials in conjunction with medical director and corporate oversight.
Education/Experience: Bachelor’s degree in Social Work or equivalent experience. 2 years of experience in managed care and/or behavioral healthcare setting. Practical knowledge of Medicaid and Health Plan administration.
Preferred Qualifications:
Must be able to work independently with minimal supervision.
3 years working in mental health-based community clinics, in-patient or out-patient treatment centers, physician office or call center environments.
3 years of member services, service coordination, or referral provider services, and working members with Medicaid and Medicare
Working knowledge of utilization review or claims processing practices in a managed care environment is preferred.
Strong communication and organizational skills is required.
Role requires strong customer services skills - outbound telephonic experience and professional phone etiquette and listening skills is required.
Proficient computer skills and experience working within large databases or provider information systems and Microsoft Office applications.
Working knowledge of behavioral health terminology
